Working inside fast-growing technology companies, one thing kept repeating: people would leave the same conversation with different assumptions. A meeting happened, but ownership stayed unclear. A document existed, but it didn't transfer the understanding behind the decision. A smart team still needed a leader to re-explain what mattered. It looked like a people problem. It wasn't.
The assumption was that this was a small-company problem — the kind of thing more process, better tools, or senior hires would eventually solve. Then the exact same breakdown showed up at enterprise scale, almost identically. More resources hadn't fixed it. That was the moment the real cause became clear: it was never about size. It was about whether the communication system had been designed — or simply inherited.
